The Sask Five Giants Association (SFGA) requires our Head Coaches, Assistant coaches, Managers and Treasurers to have a criminal record check completed every 3 years.

To complete this you will need:

• Personal Identification - Your drivers liscence photo id and health card or other form of government issue id.
• RCMP CRC Application Form (Available below).

The due date for coaches  to return documentation to SaskFive Baseball is May 8th 2026. 

Once all coach documentation has been  received SaskFive Baseball will submit to Warman/Martensville RCMP for processing.

Steps:

• Download and print the CRC Application Form.
• Completly fill out the  CRC Application Form.
• Scan CRC Application Form.
• Scan front and back of your drivers license and health card or other government issued id.
